Description:
Coordinated Care Alliance (CCA) is a network of Illinois community-based organizations. Their continuum of care services integrates social and medical aspects of care to achieve the best outcomes at the lowest costs for healthcare partners. During this webinar, participants will learn where to acquire adult protective and healthcare services using the Illinois Department on Aging (IDoA) site, community services, and supports that address the social determinants of health. Attendees will also learn about partnership opportunities available to ensure individuals remain safe and healthy in their homes.

Objectives:
At the conclusion of this learning activity, participants will be able to:

  • The IDoA and its programs keep older adults safe in the community.
  • Social determinants of health and their impact on readmissions.
  • Community programs that address the social determinants of health.
  • The benefits of the Bridge transitional care and AIMS programs on health outcomes and patient satisfaction.
