2018 Update: The Utility of Root Cause Analysis and Failure Mode and Effects Analysis in the Hospital Setting is organized by Power-Pak C.E Continuing Education for Pharmacists & Pharmacy Technicians. Goal:
Recognizing and preventing medication mistakes is a notable challenge in all healthcare settings. Pharmacists and pharmacy technicians need to understand traditional methodologies for identifying sources of risk in order to participate in error and risk researches and to realize changes in pharmacy practice.

Objectives:
At the conclusion of the session, participants should be better able to:
• Consider the prevalence of medication errors in the United States;
• Describe the utilization of root cause dissection in the healthcare setting;
• Describe the steps included in a failure mode and effects analysis;
• Review the advantages of drawing from a multidisciplinary team to complete patient safety evaluations; and
• Record 4 strategies that can be applied in the pharmacy to aid reduce medication errors.
