Conference Objectives:
Utilize evidenced based/informed TBC models to ensure patient safety, better practice, and improved health outcomes.

  • Collaborate with community and clinics to address client’s needs to provide holistic care.
  • Identify lifestyle changes that can make positive impacts for both pre-diabetes and diabetes.
  • Examine Cholesterol testing, including the lack of testing, genetic testing, and it's implications in chronic diseases.
  • Explore the development and progression of Chronic Kidney Disease related to chronic diseases.
  • Describe the latest treatment options for hypertension and diabetes including screenings and medical adherence.
