OVERVIEW

Post-Acute and Long-Term Care Medical Association (PALTmed) 2025 Annual Conference is organized by Post-Acute and Long-Term Care Medical Association (PALTmed) and will be held from Mar 13 - 15, 2025 at Charlotte, North Carolina, United States of America.

Overall Goal:
The overall goal of this program is to enhance participants’ knowledge and competencies by providing the latest developments and tools available to improve the delivery of quality care in PALTC medicine.

Learning Objectives:
Upon conclusion of participating in this educational activity, attendees should be able to:

  • Analyze available data to improve process and promote quality improvement in PALTC.
  • Discuss clinical advances and new research findings relevant to PALTC medicine.
  • Enhance skills and abilities to collaborate with and/or lead the PALTC interdisciplinary team.
  • Foster creativity and innovation in PALTC.
  • Implement Clinical Practice Guidelines (CPGs) in the PALTC setting.
  • Implement strategies for effective communication with patients, families and care teams.
  • Implement strategies to identify and address healthcare disparities in PALTC.
  • Incorporate and refine skills attitudes and knowledge to improve leadership effectiveness within a health care organization or facility.
  • Integrate knowledge and skills gained to enhance the effectiveness and quality of medical direction in nursing facilities.
  • Utilize available data, tools, and emerging technologies to inform decisions about clinical care and/or medical direction.

Designation Statements
Physician CME
Post-Acute and Long-Term Care Medical Association is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Post-Acute and Long-Term Care Medical Association designates this other/blended learning activity for a maximum of 73.5 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ABIM Maintenance of Certification (MOC)
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 73.5 Medical Knowledge MOC points and patient safety MOC credit in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

CMD Credit Hours
This CME activity has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for up to 41.5 management hours and up to 32.0 clinical hours toward certification or recertification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. Each physician should claim only those hours of credit actually spent on the activity.

The CMD Program is administered by the ABPLM. Each physician should claim only those credit hours that he/she actually spent in the activity. The maximum number of credits earned cannot exceed the maximum number of CME Credits.

    CANCELLATION & REFUND POLICY

    Attendee conference registrations are non-refundable however you may transfer from the In-Person to the Virtual Learning Track, or vice versa, at any time with no change fee.

    Only guest registration and ticketed session fees are refundable; less an administrative fee of $50. All registration cancellations must be made in writing and emailed to registration@paltmed.org by February 12, 2025, for consideration.

    Guest registration and ticketed event cancellations received after February 12, 2025, will not be refunded. No-shows will not receive a refund. Forfeited registration fees cannot be applied to subsequent PALTmed programs.

    Substitutions will be taken at any time with notification sent to registration@paltmed.org. Email must be sent by the current registrant with details including the full name of the replacement person, their credentials, mailing address, and email address. All registration details will be transferred to the new person. Substitution requests received by February 12, 2025, will be processed without charge. Substitution requests received after February 12, 2025, will incur a $25 processing fee.
